*There is a special discount if an office sends more than four staff and the doctor also attends.  The doctor’s fee is only $200.

*Cancellations: You may request a cancellation by submitting written notice or by phone to PUMC 32+ days prior to each seminar.  A refund, less a $75 service charge, will be remitted.  Cancellations less than 32+ days prior to the seminar shall forfeit one half of the registration fee.  No refund will be extended 10 days prior to the seminar.
